Considering the quality of care from views of patients is an important aspect in improving health services and expected value of patient. So the value chain is a strategic tool in health care organizations to create value and achieve the goal of continuous improvement in quality. In this paper we discuss the role of value chain in promoting quality of health care. This study was review which was done the first half of 2012. For searching related articles and documents, the main value chain keyword and sub keywords quality and satisfaction were used in the database. Title, abstract and full text investigated respectively, and then 43 of them selected for evaluation. Findings were classified in several parts. These findings include the concept of quality, planning achieve to value-based quality, value chain, its use in health care organizations, describing components, logic planning implementation strategies, value adding healthcare service delivery strategies and value adding support strategies. In this paper, we try to noted different aspects of the value chain, two important parts of value chain including value adding healthcare service delivery strategies and value adding support strategies. Value chain can be an important tool for managers in order to planning and improve the quality. Keywords: Quality of Health Care; Health Care; Value Chain; Patient Satisfaction.

Introduction: On one hand, risk management is capability of recognition available risk factors. And on the other hand it is risk analysis and the selection of appropriate strategies to control and eliminate it. The purpose of this study is to identify, assess and provide appropriate actions to control, reducing and eliminate risks in the ICU ward in Azahra hospital in 2012, with the FMEA technique.Method: The present study is a qualitative research conducted in 2012 through direct observation, interview with the officials and authorities in the ICU ward, and available document review. The participants include nurses who identify the process of ICU ward. 4 people were selected purposefully. To collect the data, the standard worksheet of the failure mode effect analysis (FMEA) technique was used.Results: The finding suggested that 58 failure modes were identified in the ICU ward in Alzahra hospital in Isfahan from among which 13 potential failure modes were related to general practices, 5 cases to neurological care, 8 cases to gastrointestinal care, 6 to the sampling, 5 to the medication, 4 to skin care, 17 to respiratory care. Of course some errors were discovered were common.Conclusion: According to the findings, several causes underlie errors in nursing cares. Overuse of nurses by setting the sequential shifts and rely on nurses who have been trained under and also stressful work environment shortage of qualified and expert nurses, endanger the patient's health due to the possibility of errors in nursing work. The best solution is to promote the improvement of hospital staff. The results of this study can be nurses and nurse managers are made aware of the dimensions underlying operating errors and a new approach to planning and greater attention to patient safety and Health Maintenance can be explained.Key words: Evaluation; Risk; Intensive Care Units; Hospitals.

Introduction: Human skills, knowledge and efficiency are important factors in the success oforganizations and community health centers which have a critical role in total qualitymanagement (TQM) as a management system. Knowledge is necessary as a source for theorganizations survival. On the other hand, in order for implementation of knowledgemanagement, there is a need for a relatively long period, providing intellectual and culturalcontexts, educational skills and knowledge and technological centers. This study aimed todetermine the relationship between TQM and knowledge management from the perspective offaculty members of Isfahan University of Medical Sciences, Iran.Methods: This was a descriptive-correlational study. The study subjects included the total offaculty members of University of Isfahan (491 people) and Isfahan University of MedicalSciences (594) during academic years 2009-2010 who were selected through proportionatestratified random sampling (300). Data collection tools included a Baldrige questionnaire basedon the TQM model and knowledge management components based on Pastor standardquestionnaire. The content and face validity of questionnaires were confirmed and reliabilitythrough Cronbach's alpha coefficient were obtained as 0.87 and 0.91, respectively.Results: Mean of all the TQM criteria were higher than average in University of Isfahan exceptleadership, strategic planning and human resource management. It was the same in IsfahanUniversity of Medical Sciences except criteria like focusing on customer, information analysis,strategic planning and process management. All the components of knowledge management weremore than average level in Isfahan University of Medical Sciences, and except refining theknowledge in University of Isfahan. Correlation coefficient between scores of TQM and knowledgemanagement components was significant (P ≤ 0.05). In addition, multiple regression showed thatthe correlation between TQM and knowledge management was statistically significant.Conclusion: TQM criteria can be effective in line with appropriate functioning of knowledgemanagement and its impact on organizational performance can also increase the efficiency of theorganization

Introduction: The concept of organizational learning capability (OLC) emphasizes theimportance of facilitating factors for organizational learning or the organizational propensity to learn. Consequently, measures of OLC have traditionally determined their dimensions or facilitating factors based on the literature. The purpose of this study was to validate and evaluate the factorial construction of OLC among the faculty members of Isfahan University of Medical Sciences, Isfahan, Iran.Methods: This was an applied, psychometric, cross-sectional research during the educational year of 2010-2011. The study sample consisted of 220 randomly selected faculty members ofIsfahan University of Medical Sciences. Data was collected by the OLC scale developed by Chiva et al. in 2007. The collected data was analyzed by descriptive and inferential statistics using SPSS 18 . Confirmatory factor analysis was also conducted in LISREL8.5 . Results: The model had an acceptable fitting for purpose based on GFI, AGFI, RMSEA, and CFI indices. All the estimated parameters had statistically significant effects on their related factors. The model enjoyed an appropriate level of reliability (total reliability = 0.93). The convergent validity between factors of the scale was confirmed by using correlation coefficients (0.41 to 0.75). In addition, table of norms (Z and T) provided crude and ranking scores.Conclusion: The confirmed measurement scale for organizational learning capability could be implemented as an audit tool. Thus, presidents, deans and managers of departments could unveil weak and strong organizational learning issues. Such a process would finally lead to improved performance of the university.

Introduction: Hospitals are responsible to respond to legitimate demands for health information release while protecting the confidentiality of patient health records. This study investigated and compared laws and policies of health information disclosure for legal authorities in Iran and some selected countries. Methods: This descriptive-comparative study included health information disclosure laws and policies in selected countries such as the United States, Australia, England, Malaysia and Iran. Data were gathered from systematic internet search, library resources and communication with health information professionals. Data analysis was done using comparative tables and qualitative method. Results: The results showed that in all the five studied countries, there are specific written policies and procedures for health information disclosure for legal authorities. It was found that, without a judicial warrant, a patient consent is needed for medical record disclosure for courts, representatives of judicial authorities and law enforcement agencies. Conclusion: In all of the selected countries there are ordained limitations for health information release to protect patient rights. Keywords: Disclosure; Legislation; Medical Records.

One of the ways to manage human resources is performance management. With a systematic approach, this sort of management determines strategic goals, identifies indexes, collects, analyzes and reports data and ultimately improves the organization performance. Performance management is a process that calls for the interaction of factors such as administrative goals, accountability standards and evolutionary behaviors.
There is a variety of models in performance management. However, designing and establishing performance management plan require us to follow a given practical model. The selection and implementation of performance management models can enhance the accuracy and reliability of the process itself. Among the models, the following can be referred to :Canada auditing office, World Health Organization, European foundation for quality management and input-output linear model. This study is aimed to evaluate these models to help us acquire a precise understanding of them and their impact on the performance of an organization.
